Abstract

Non-alcoholic Fatty Liver Disease and Type 2 Diabetes mellitus are closely linked metabolic disorders related with worsening of liver injury and fibrosis. The current study aimed to asses' serum cytokeratin 18 and aminotransferase ratio as indicators of liver fibrosis progression in patients with T2DM and NAFLD. The study was conducted involving 180 participants aged range between (40–70) years old. The contributors were divided into four groups. Auto Chemistry Analyzer Cobas C311 was used to measure serum AST, ALT, ALP, FBS. Sandwich enzyme-linked immunosorbent assay was used to measure serum CK18 and insulin. AST/ALT ratio was calculated mathematically. A significant difference was observed in BMI, ALT, AST, ALP, FBG, insulin and CK18 levels in patients group compare to control group. The upper values of CK18 observed in the DM+ NAFLD group (3.09 ± 1 U/mL). The sensitivity and specificity of CK18 were 82.5% and 65.25%, respectively, according to the receiver operating characteristic curve. CK18 may be considered a valuable non-invasive biomarker for monitoring hepatic fibrosis progression in patients with T2DM and NAFLD. In contrast, the AST/ALT ratio showed limited diagnostic utility (P = 0.683).
